Discharged

Clients who have been hospitalized are often discharged after short or long stays and may still be
acutely ill or needing more care as usual, Clients who have been hospitalized for activity or
mobility problems often need continued care in the home this becomes a challenge to the family.

Service area and description

Personal Care Services:

Complex Care Services (RN or Delegated to Caregiver under supervision)

1. Vital signs monitoring
2. Medication assistance
3. Closely link patient with the caring RN (If delegated task)
4. Appointment to the treating doctor
5. Therapeutic massage

Physical fitness (When applicable)

1. Aerobic exercises, (Walking, Running, )
2. Muscle strengthening exercises, (Lifting weights, jumping rope, ), 3. Balance training exercises (Walking backwards or sideways, standing on one foot while doing an upper body muscle-strengthening activity, such as bicep curls),
4. Joint Flexibility exercises(ROM,)
5. Functional exercises (Tandem and one-leg stands, squatting, chair stands, toe raises, and stepping over obstacles). Our Nurses often teach family members or caregivers safe moving, lifting, and transfer techniques in the home setting
